A Local Police Records check form for the purpose of a Record Suspension Application (formerly known as Pardons) may be made in person at 2670 Queensview Drive during our regular business hours.  At this location, you will require:

  1. Your Local Police Check Form (page 1-2)
  2. A list of your convictions provided by the RCMP and/or private sector (not provided by the Ottawa Police Service)
  3. A copy of your Fingerprints taken by a private sector (service not offered by the Ottawa Police Service)
  4. Two (2) pieces of valid Identification.  One piece of identification must contain your photo and date of birth on it.
  5. A Fee payment of $63.

***Accepted methods of payment are: Cash, Debit, Certified Cheque, Money Order, Visa, MasterCard, or Amex***

A Local Police Records check form for the purpose of a Record Suspension Application may be submitted by mail to:

Attention: Record Suspension Application

And will require the following for completion:

  1. Your Local Police Check Form (page 1-2)
  2. A list of your convictions (not provided by the Ottawa Police Service)
  3. A copy of your Fingerprints taken by a private sector (service not offered by the Ottawa Police Service)
  4. Copies of two (2) pieces of valid Identification
  5. A Certified Money Order or Certified Cheque of $63 paid to the City of Ottawa
  6. A Consent Form from an official agency acting on your behalf allowing us to send the results to them (if applicable)
  7. A return mailing envelope including address and postage stamp

How long does my Local Police Records Check Form for the purpose of a Record Suspension Application take to be completed?

  • In person: may be completed at the time of your visit as long as all necessary documentation is presented at the time of application
  • By mail: It may take up to ten (10) business days
