Record Suspension Application

The Customer Service Desk at 2670 Queensview Drive is CLOSED for all walk-in service.

A Local Police Records check form for the purpose of a Record Suspension Application may be submitted by mail to:

Attention: Record Suspension Application

You require the following for completion:

  1. Your Local Police Check Form (page 1-2)
  2. A list of your convictions (not provided by the Ottawa Police Service)
  3. A copy of your fingerprints taken by a private sector (service not offered by the Ottawa Police Service)
  4. Copies of two (2) pieces of valid Identification
  5. A Certified Money Order or Certified Cheque of $65 paid to the City of Ottawa
  6. A Consent Form from an official agency acting on your behalf allowing us to send the results to them (if applicable)
  7. A return mailing envelope including address and postage stamp

How long does my Local Police Records Check Form for the purpose of a Record Suspension Application take to be completed?

  • By mail: It may take up to ten (10) business days
